Squadz Waiver of Liability

COVID-19 Disclaimer:

The health and safety of our members and guests is our highest priority. While our policies are designed to help protect you, Squadz or any affiliates cannot remove all risk or otherwise guarantee or promise that you will not sustain any injuries or damages from viruses, communicable diseases, or other health hazards associated with your use of Squadz or any affiliates's premises, facilities, equipment, services, activities or products, including exposure, transmission, infection, illness, sickness, disease or death with respect COVID-19 and the virus that causes it, SARS-CoV-2. Because our facilities and activities provide opportunities for vigorous higher-intensity exercise, you may experience or encounter more significant respiration than in other locations. To help protect our community, we encourage you to refrain from the use of Squadz or any affiliates's premises and services if you have symptoms of COVID-19, maintain social/physical distance at all times from others with whom you do not share regular contact, and subject to individual health conditions use a barrier mask within our premises.

Model Release:
I release the photographer and Squadz from liability for any violation of any personal or proprietary right I have in connection with any reproduction of or use of photographs in which I appear. I consent to the reproduction or use of photographs, without my name (or other registered participants), taken of me while participating in Squadz recreation programs.  Squadz and its photographer will only take photographs and videos in a public setting in which there is a reasonable expectation of privacy.

Waiver of Liability

I/We hereby understand and acknowledge that the training, programs, use of equipment and events held by Squadz or any affiliates may expose me to many inherent risks, including accidents, injury, illness, or even death. I/We assume all risk of injuries associated with participation including, but not limited to, falls, contact with other participants, the effects of the weather, including high heat and/or humidity, and all other such risks being known and appreciated by me. 

I/We hereby acknowledge my responsibility in communicating any physical and psychological concerns that might conflict with participation in activity. 

I/We acknowledge that I am physically fit and mentally capable of performing the physical activity I choose to participate in. After having read this waiver, the COVID-19 Disclaimer, the Model Release, and knowing these facts, and in consideration of acceptance of my participation and Squadz or any affiliates furnishing services to me, I agree, for myself and anyone entitled to act on my behalf, to HOLD HARMLESS, WAIVE AND RELEASE Squadz or any affiliates, its officers, agents, employees, organizers, representatives, and successors from any responsibility, liabilities, demands, or claims of any kind arising out of my participation in Squadz or any affiliates programs, use of any equipment, and/or events. By acknowledging this agreement, I/We indicate that I/We have read and understand this Waiver of Liability, the COVID-19 Disclaimer, and the Model Release. I/We am aware that this is a waiver and a release of liability and I voluntarily agree to its terms.
